A THC/CBD capsule just aced a real dementia trial — nearly 9 in 10 patients improved

The most rigorous cannabinoid result of the year didn't come from the industry — it came from hospice care. Topline results from the LiBBY trial, presented at the Alzheimer's Association International Conference, show a purified oral formulation of 2mg THC and 100mg CBD, taken twice daily, significantly reduced agitation in hospice-eligible patients with late-stage dementia. This was a Phase 2, multicenter, randomized, double-blind, placebo-controlled trial — the design skeptics keep correctly demanding — with 120 participants averaging about 80 years old.

The numbers are striking. At week two, the treatment group's agitation scores dropped 6.27 points more than placebo, with 83.9% showing clinical global improvement versus 30.5% on placebo. By week 12 the gap had widened: an 8.23-point difference, with 87.2% of treated patients improved against 23.6% for placebo. Serious adverse events ran higher in the treatment arm — 23.3% versus 11.9% — though investigators judged none of them medication-related, a determination worth holding loosely until the full data publishes.

Context makes this bigger than one result. Dementia-related agitation is one of the hardest symptoms in end-of-life care, the trial was funded through the NIA-backed Alzheimer's Clinical Trial Consortium rather than a cannabis company, and 58% of participants came from underrepresented racial and ethnic groups — a population cannabis research has chronically failed to enroll. It also demonstrates that late-stage dementia patients can be safely enrolled in cannabinoid trials at all, which clears a path for the follow-up studies this will inevitably spawn.

For the medical side of the industry, this is the citation you'll be hearing for years — a defined formulation, a real endpoint, public funding, and a placebo-controlled win. It is not a license to market dispensary products for dementia; the tested ratio (heavy CBD, whisper of THC) looks nothing like most retail shelves. The lesson runs the other direction: precise formulations, tested honestly, is what moving cannabis science forward actually looks like.
